Where is the House of Tribes?

I’m planning a trip to New York and want to hit up some jazz clubs, big and small. One of the smaller ones I’d like to visit (even if nobody is playing while I’m there) is the House of Tribes, but I have no clue where it is. I’ve tried Googling it and it shows about 5 million little dots all over NY, none of which actually say House of Tribes.

So can anyone help me locate it??
